From screening to clarity

How one practice helps catch hidden diagnoses
The client seemed straightforward. Good grades. Holding it together. A typical intake.
Then the assessment results came back, and Steve Monte, founder and Clinical Director of Rock Life Counseling in Indianapolis, saw something completely different: bipolar disorder with manic features. Substance use in the past month. A pattern of risky choices that could've spiraled for months or years without intervention.
"Something that could've easily taken three to six months to fully uncover," Steve said. "We were able to get to it within the first three or four visits."
That's not luck. That's what happens when you have the right tools and the right approach.
The gap nobody likes to talk about
Most mental health practices operate the same way: a clinician uses a few screeners. Pulls in a few assessments. They're all validated. They take a while, so you assume they’re catching everything.
At Rock Life Counseling, Steve and his team were experiencing exactly that problem. Before they found the K-CAT®, they were "piecing things together"—one questionnaire at a time, one issue at a time. It felt thorough, but it really wasn't.
"There was a good chance they could miss a significant issue," Steve explained. "ADHD is one that comes to mind when that wasn't the clinical presenting issue from the family. Inattentive ADHD particularly, or a bipolar disorder could be really easy to miss in sort of the noise of the anxiety and/or depression that often come up."
That noise matters. It costs time. It costs accuracy. And most importantly, it costs your clients the early intervention they need.
What changed with the K-CAT®
Five years ago, Steve's practice began using the K-CAT®, a computer adaptive testing platform designed to assess for multiple diagnostic categories—depression, anxiety, ADHD, bipolar disorder, substance use, and more—all in one comprehensive assessment.
Here's what makes it different: instead of asking the same standard questions every client answers, the K-CAT® adapts. Each response shapes the next question. The result? You get precision without burden.
"We were looking for options and what we found was there weren't really any options that used computer adaptive testing besides the K-CAT®," Steve said. "It's sort of first in class, best in class from our perspective, and being able to assess for eight or ten different diagnostic categories and to do that really thoroughly without a high burden on the time and the energy of both the kids, the teens, and then their parents as well, filling out those questionnaires."
Speed and accuracy aren't usually a package deal. Until now.
Life before the K-CAT®:
- Three hours of interviews that exhausted already distressed clients.
- Questionnaires that barely scratched the surface.
Life with the K-CAT®:
- Screen each client in minutes.
- Use any device.
- Choose English and Spanish.
- Send the questionnaire to families to complete on their own schedule.
And the clinician side? Just as clean. No redundant questions. No wasted time. Real precision.
"We know we're getting the accuracy and the precision of a large item bank, but it's all boiled down into that computer adaptive testing element," Steve continues. "It's condensing the time that it takes and making it convenient for everybody."
The case that changed everything
One client showed up presenting as "pretty straight laced and successful." Good student. Doing well. Mild anxiety, mild depression.
Steve gave him the K-CAT® anyway. The results immediately flagged something he wouldn't have thought to ask about in a traditional intake: Substance use. Marijuana. Alcohol. Cocaine in the last 30 days.
"The cocaine use was something I probably wouldn't have thought of asking in the middle of trying to get a lot of information in a short amount of time during the initial assessment process," Steve admitted.
Digging deeper revealed the story: partying with the wrong crowd, making bad choices that didn't match who this teenager usually was. And beneath all that—the real issue—were manic episodes of bipolar disorder.
"What presented as some simple mild anxiety and depression was really a bipolar disorder that was having some functional impairment, really significantly impacting the choices this young man was making related to substance use."
A traditional intake? Three to six months to uncover all this. Maybe longer.
With the K-CAT®? Three or four visits.
Real progress you can see
Rock Life Counseling uses the K-CAT® for more than just initial diagnosis. They use it to track progress over time, but their clients aren't filling out the same checklist over and over. They're getting fresh questions each time, which means the data they get back is genuinely revealing how much things have shifted.
"We've retested after a bit of intervention and clinical work and have gotten some numbers that showed them their depression really had lightened, or their anxiety is significantly less than it used to be," Steve said.
That matters. Therapy can feel fuzzy. "Am I actually getting better?" is a harder question to answer than it should be. But numbers? Data? That gives clients something concrete to hold onto.
"It's really been a good tool at times for our therapists to take a bit of a vague story about the improvements and then anchor them with some specific numbers and ratings of how much better are you doing than this previous point in time when you came to us and you were really struggling."
Why this works for complex cases
Rock Life Counseling isn't just treating anxiety and depression. They're working with kids dealing with sleep issues, learning disabilities, nutrition challenges, and trauma. Sometimes it's hard to know which pieces are mental health, and which are something else entirely.
The K-CAT® helps you sort through the noise.
"K-CAT® has really been helpful in allowing us to sort through a number of different complex variables and get to what are the most important, what are the most significant and severe issues that we need to address," Steve explained.
That's not just faster. That's smarter. That's meeting families where they actually are instead of where you assumed they'd be.
What matters in the end
- More accurate diagnosis.
- Faster time to real treatment.
- Less time chasing the wrong problem.
- Less time screening and more time helping.
- Clients who walk away knowing—not guessing—that they're getting better.
"[The K-CAT® has] allowed us to get to that endpoint quicker," Steve said. "And I think it gives clients some confidence that they really are doing better. They know, and they walk away with some increased confidence in the progress that they've made for themselves."
That confidence changes everything.
