The Personal History Survey (PHS) is a web-based, branching survey instrument designed to gather information relevant to a respondent’s psychological suitability for employment in a public safety position. The PHS Report is generated from a respondent’s completed PHS and is a narrative-form report intended for use with police, correctional, dispatcher, firefighter/medic, security, and other public safety, public trust, and safety-sensitive positions. The PHS can be used as a standalone instrument or alongside the MMPI® and MPQ® as part of a comprehensive preemployment psychological evaluation. 

The PHS can be used: 

  • As part of a standard assessment battery for conducting preemployment psychological evaluations of public safety applicants or candidates  
  • To guide the review of personal history information and the postoffer clinical interview 
  • By background investigators (preoffer version) to aid in organizing and exploring job-relevant, nonmedical personal history

  • Includes positive and negative history The PHS supports psychological screening focused on risks of counterproductive behaviors and negative outcomes and on traits and characteristics that may mitigate risk and contribute positively to work performance.
